The Welfare Fund has completed the 2023 filing of Form 1095-B, but we will not be mailing out the forms to participants this year as they are not required for you to file your tax return.
If you have any questions or would like to receive a copy of your 1095-B, please call the Welfare Fund Office at 1-800-704-6270, email 701claim@mech701-benefits.org, or mail a request to Automobile Mechanics’ Local #701 Welfare Fund, 361 S Frontage Road, Suite 100, Burr Ridge, IL 60527.
