| Accidental Death and Dismemberment (AD&D)
AD&D benefits are paid if you sustain an accidental injury that results in your death or loss of limb or eyesight within 90 days after an accident. Note that AD&D death benefits are in addition to the death benefit described
on page __. AD&D benefits are not available to employees covered under COBRA Continuation Coverage or dependents.
Benefit Amount
The amount of your AD&D benefit is based on the type of loss, as shown below.

Note that benefits paid for all losses from a single accident are limited to the payment for the most severe loss. Loss of hand or foot means the complete severance at or above the wrist or ankle joint. Loss of eyesight means the total
and irrecoverable loss of the entire sight of the eye.
Beneficiary
Generally, all benefits are paid to the employee. However, in the event of your death due to an accident, AD&D benefits are paid to your beneficiary¾the person you have named to receive your death benefit. You name a beneficiary when you
join the Plan, and you may change your beneficiary at any time by completing and signing a new history card. For more information on payments to beneficiaries, see Beneficiary on page __.
Exclusions And Limitations
AD&D benefits are not paid for any loss due to:
Bodily or mental infirmity, sickness, or disease of any kind.
Intentional self-destruction or self-inflicted injury.
War, whether declared or undeclared, or any act of war.
Participation in an act that is in violation of a state or federal statute.
